IMPORTANT The trial license agreement must be signed by someone who is authorized to sign legal software license agreements in your department (e.g. PI, Department Head, or Department Administrator).

How to Complete the License Agreement

Page One

Please enter your name and address on Page One.

Page Five

Please read the agreement and then sign and enter your name, title, and date. Please also sign the section entitled Acknowledgement by Responsible Scientist. If you are the PI of the lab then you would sign both places on this page.

Page Six

Please enter your name or organization (e.g. Company name or University name ). Please enter the name of the product and modules you wish to evaluate (e.g. ICM-Pro + Homology) and insert the platform you will be testing it on (e.g. Windows, Mac, or Linux). Please read the agreement and then sign and enter your name, title, and date.
